Worship Service Nursery Registration: New Guest or Updated Registration

Please complete this form to register your child for nursery care in the upcoming worship service. Our nursery is equipped for children up to four years old, but if your child is older than four and would still benefit from nursery care instead of children's church, please let us know in the last comment box so that we can speak with you about accommodations.

Snacks:

Snacks may be provided during the worship service. Please note whether your child is old enough to safely consume a snack. Typical snack selections include goldfish, teddy grahams, cheerios, etc.

If you have packed a specific snack for your child, please note that if another child has an allergy to its ingredients, we may not be able to let your child consume that snack during nursery.

If your child has an allergy that is severe to the point where he/she cannot be in the same room with another child consuming that allergen, please indicate below!

Diaper Changing & Potty Training Policy:

Please note that at this time we are not able to change diapers during the service. If your child needs to be changed, we will text the number provided in this registration form or will come find you in the sanctuary. Parents may use the family side (the drop-off side) of the nursery for use of the changing table, to utilize a rocking chair to nurse their baby, or to play with a fussy child. For the safety of all children under our care, no adult, youth, or child is permitted past the knee-wall of the nursery unless scheduled to serve that day.

Please ensure that your potty-trained children have used the bathroom prior to drop-off and that your diapered babies have been recently changed. If your potty-trained child needs the bathroom, we will walk them to the bathroom and ensure it is completely empty before they enter. It is preferable that your child is able to completely take care of themselves in the bathroom, but we can text you to meet your child in the bathroom or can assist the child if you specifically release nursery workers to do so.
